Extended phase III results support nivolumab as adjuvant SoC in MIBC

Neoadjuvant systemic therapy plus radical cystectomy is the current standard of care (SoC) for patients with muscle-invasive urothelial carcinoma (MIUC), including muscle-invasive bladder carcinoma (MIBC). However, patients with MIUC remain at high risk of lethal metastatic recurrence. At the 28^th Annual Scientific Meeting of the Hong Kong Urological Association, Professor Jürgen Gschwend of the Technical University of Munich in Munich, Germany, discussed extended follow-up results of the CheckMate 274 trial, which demonstrated continued survival benefits with adjuvant nivolumab in patients with high-risk MIBC.

Patients with relapsed or refractory diffuse large B-cell lymphoma (r/r DLBCL) have poor outcomes with traditional therapies. However, phase 3 clinical trials of tisagenlecleucel, a CD-19 directed chimeric antigen receptor T-cell (CAR-T) therapy, have shown improved ORR, CR, PFS and OS in patients with r/r DLBCL, giving hope to these patients. Long-term real-world data from Hong Kong has also shown improved outcomes with tisagenlecleucel in these patients.

At a symposium hosted by AstraZeneca on 20 April 2023, a panel of distinguished experts discussed the latest developments in the management of early-stage epidermal growth factor receptor (EGFR)–mutated (EGFRm) non–small–cell lung cancer (NSCLC). Speakers included Dr Vincent Ng, who highlighted the advances in surgical interventions, and Professor Masahiro Tsuboi, who addressed unmet clinical needs in early-stage EGFRm NSCLC and potential solutions such as osimertinib as an adjuvant treatment. The symposium featured a lively debate on the utility of disease-free survival (DFS) as a surrogate endpoint and concluded with an informative panel discussion.

Glioblastoma multiforme (GBM) is the most common and aggressive brain tumour with a very poor prognosis. Tumour-treating fields (TTFields) are a relatively new, noninvasive treatment for GBM, which delivers low-intensity alternating electric fields that disrupt cancer cell division. At a recent scientific meeting organized by the Hong Kong Neurosurgical Society, Dr Alexander Grote of the Department of Neurosurgery, University Hospital of Marburg, Germany, and Professor Michael Lim of the Department of Neurosurgery, Stanford University, US, discussed the evolving role of TTFields in managing patients with GBM.

Mutations in the spike protein of the virus have increased its ability to infect humans and evade neutralization by natural or immunisation-induced antibodies. The highly mutated Omicron variant of SARS-CoV-2 has multiple sublineages with varying transmissibility and immune evasion potential. Pre-exposure prophylaxis plays a crucial role in protecting the immunocompromised populations against severe disease; however, the rapidly changing landscape of the variants poses a major challenge to the current and future effectiveness of pre-exposure prophylactic monoclonal antibodies.

A 60-year-old female complained of dizziness and weakness over the left side of her body (assessed strength, 4/5) when she presented in May 2019. CT scan of the brain revealed a mass measuring approximately 5 cm in the right temporal region and some compression of the ventricles. Craniotomy to excise the suspected malignant brain tumour was carried out in the same month, but was incomplete. The patient’s preoperative Eastern Cooperative Oncology Group (ECOG) performance status (PS) was 1.

Intensive care unit (ICU) patients are at risk of developing serious infections with multidrug-resistant organisms (MDROs), which require appropriate and adequate antibiotic coverage. Early empirical coverage is pivotal in saving patients’ lives. At a recent webinar co-organized by the Society of Infectious Disease (Singapore) and Pfizer, renowned Professor David Paterson, Professor of Medicine and Director, The University of Queensland Centre for Clinical Research, and Consultant Infectious Diseases Physician, Royal Brisbane and Women’s Hospital, Brisbane, Australia, discussed the role of newer antimicrobial agents, including ceftazidime-avibactam (Zavicefta) in the management of MDROs in the ICU. Dr Wong Sin Yew, Infectious Disease Physician at Gleneagles Medical Centre and Mount Elizabeth Novena Specialist Centre, Singapore, chaired the event.
