True hypocalcemia is a decrease in ionized calcium, while factitious hypocalcemia is a decrease in total calcium but not in ionized calcium.
Symptoms such as tetany, paresthesia, muscle cramps and spasms, and seizures are observed only in those with decreased ionized Ca concentration.
Laboratory tests include serum calcium, although ionized calcium is the preferred measurement, serum albumin, and phosphorus, PTH, and Mg if available.
Treatment will depend on the severity, cause, presence of symptoms and how rapidly the hypocalcemia developed.
